Important - All Outdated Form Applications will be Returned
Please be aware that as at June 1 2014, the Ministry updated the application forms for obtaining a Security Guard and/or Private Investigator licence under the Private Security and Investigative Services Act, 2005 (PSISA).
If you have not already done so, please replace all previous versions of the form with the updated versions listed below. Applicants who have submitted outdated application forms will have their application returned.

The above forms are also online through ServiceOntario. Online applicants can also:
- Apply for a new licence or renew their existing licence to obtain a digital licence
- Resubmit an application which may have been returned
- Check the status of their application
Returning applications is an inconvenience to the Applicant and can impact the ability for the Ministry to process a correct and complete application in a timely manner. We appreciate the assistance of licenced agencies and registered employers to do their part to ensure their licenced personnel use the correct application forms.
